What is semaglutide?

Semaglutide is the newest GLP-1R agonist and was approved by the FDA in late 2017 as a once-weekly injection for the treatment of type 2 diabetes. The peptide is chemically similar to liraglutide but contains two structural modifications (Lau, Bloch, et al., 2015). First, glycine is replaced at position 2 with the non-proteinogenic amino acid 2-aminoisobutyric acid (Aib). Second, octadecanedioic acid is linked to the side chain of lysine-26 via a short polyethylene glycol (PEG) spacer and a gamma-glutamic acid linker. The amino acid substitutions render the peptide more resistant to DPP-4 than liraglutide, and the presence of the 18-carbon fatty acid moiety results in a high binding affinity to serum albumin, which translates into a half-life of approximately 7 days in humans.
